You must choose one sport as your track for the week. Changes to your choice will only be considered on a first come basis as space is available.

As the camp will have kids with various levels of sports skill, all effort will be made to mix the teams so everyone has fun, and to have competitive parity for each team.

PERMISSION & WAIVER.

By selecting "yes" below I agree that:

I hereby give consent for my child under my care, named above, to attend and participate in Sports Camp 2026 at Calvary Pentecostal Church from on Monday July 20^th to Friday July 24^th, 2026.

I understand that my child will be participating in non-contact, non-rep sports and that there is a degree of inherent risk of injury or death involved with these, or any sport or activity. I release Calvary Pentecostal Church, their officers, board, staff and volunteers from any liability in the unlikely event of severe injury or death to my child. I understand the camp is taking all necessary precautions to provide the utmost safe playing environment.

I (we) understand that in the event that medical treatment is required every effort will be made to contact me. However, if I cannot be reached, I give permission to give a licenced medical practitioner permission to give the necessary care for my child’s physical health and wellbeing, including the care of the onsite nurses.

I give permission for my child’s picture or video to be taken for the purpose of daily and final day slide show presentation of the week’s event that could be posted online on the church’s website or social feeds.

I agree to fill out and sign a physical copy of this form at registration on the first day of camp.
